What you will do
* Conduct comprehensive assessments in line with the Care Act 2014 and the Mental Capacity Act 2005, ensuring the best outcomes for adults in the community.
* Manage safeguarding concerns and enquiries in accordance with Section 42 of the Care Act, ensuring the safety and well‑being of vulnerable individuals.
* Navigate the CHC processes, including completing checklists and participating in MDT/Decision Support Tool meetings.
* Collaborate effectively with health and social care colleagues, contributing to a dynamic and balanced team.
* Utilise your initiative to drive positive outcomes and advocate for adults within the North Community Mental Health Team.
