You will be working with apany that provides amodation with support for 16–19-year-old looked after children and care-leavers, to enable them to live semi-independently.
Young people who use the service may have a range of support needs. These can include needing to access-
1. Education and/or employment.
2. Mental health or other health services, registering with GP, dentists and opticians
3. Programs such as Youth Prevention, Drug and Alcohol Services.
4. Applying for documents such as a passport, driving licence, bank accounts
5. Learning how to food, cook healthy meals, use utilities such as a washing machine, learning how to budget effectively

7. 24 and 48-hour shifts
8. Day shifts
9. Sleep in nights
10. Constantly assessing the young person’s support plans, health plans and risk assessments.
11. Ensure that young person’s care plans are followed so young people have as much control over their future as possible
12. Assist young people to maximise their finances effectively and to develop their budgeting skills
13. Ensuring that all young person’s personal records are kept up to date
14. Accuratelypleting the young person’s daily logs
15. Attending meetings with young people and other inter-agencies
16. Add young people’s appointments to the young person’s noticeboard and to theputerised calendar. To inform management of important meetings
17. To give a written handover to the next staff member on shift
18. To make management aware of any new risks / concerns to/about young people or staff
19. Ensure that the fire alarms have been tested each week and recorded
20. Assisting young people to create CVs and to find employment
21. Encouraging young people to engage with education and to assist in finding relevant educational opportunities
22. Make sure the young person’s medical and health needs are met by assisting with medical, dental, optician appointments.
23. Assist young people to develop menu plans, cook healthy meals and food shopping
24. To promote independent living, build confidence and self-awareness
25. To be a positive role model and be able to enforce firm but fair boundaries
26. Taking young people to family contact, meetings, reviews, appointments, interviews
27. Enable young people to engage in appropriate activities and to be creative and inspiring for young people to start new interests and hobbies
28. Provide quality, responsive support that meets the physical and emotional needs of every young person
29. Be aware of policies & procedures and pay particular attention to safeguarding, bullying, equality and diversity and health & safety.
30. Keep professional boundaries in and outside of the work environment
31. Partake in the general upkeep of the house – gardening, light cleaning, decorating.
What you'll need to succeed
32. Enhanced DBS
33. Driving License and access to own Vehicle
34. Experience working within supported or sheltered Housing
35. Knowledge on support plans
36. Knowledge of safeguarding
