Community Nurse

An exciting opportunity has come up to joinour busy Neighbourhood Team. We are looking for enthusiastic, hardworking, Band5 Community Nurses to work 30 or 22.5 hours per week. Community Nursing is an integralpart of the NHS 5-year plan with an emphasis now being placed on admissionprevention.

You will need to be passionate about communityservices and shaping a new way of delivering care.

We are an integrated team who workcollaboratively and compassionately with therapists, local authority services,the urgent care response team and our colleagues from The Primary Care Networkto provide patient centred, responsive care which meets the complex and varyingneeds of our housebound community.

Main duties of the job

The role requires

Theability to communicate effectively at all levels

Flexible workingto provide care across the local area (the service operates from 7:30am 6:30pm,Monday - Sunday incl bank holidays)

Anenthusiastic, motivated and innovative thinker.

Theability to work autonomously, manage your own case load and work within a multi-disciplinaryteam

Theability to problem solve

Experienceof working in a fast paced and varied environment

Full UKdriving licence and use of car.

Your typical working day willinclude home visits for adults over 18 years needing a range of nursinginterventions, including, but not limited to end-of-life care, all types ofwound care, IV therapy, PICC line care, catheter care. You will be responsiblefor holistic assessments of patients taking into consideration theirenvironment and social circumstances.

Youwill support in the education of student nurses, both pre and post registration,as well as nurse associates and health care assistants. As a band 5 nurse, youwill oversee patients and assist in decision making regarding their care. Youwill report to your immediate Band 6-line manager and have the support of thewider leadership team. You will also attend daily handovers and participate inMulti-disciplinary Team meetings.

Job responsibilities

The post holder will:

Behave consistently with thevalues and beliefs of the organisation.

Act as a role model to colleagues,always seeking to maintain the highest standards of professionalism.

Use initiative and take responsibilityfor themselves and the quality of their work, documentation and the servicethey provide to patients.

Work as an autonomous practitionerwithin the wider integrated team providing rapid and planned assessment andtreatment interventions to patients in their own homes or other communitysettings. Promote independence and maximise independence, health and wellbeingwith a focus on patients remaining in their own homes for as long as possible.

Regularly take responsibility forthe caseload in the absence of the Senior Community Nurse.

Person Specification


Additional Criteria

* Self-motivated and able to motivate others

* Able to meet the travel requirements of the role
* Willingness to undertake further training


Qualifications

* Registered Nurse (RN1)
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
* Accredited courses in palliative care, tissue viability, Diabetes
* Practice Assessor training


Experience

* Experience gained during Pre-Registration training
* 2 years post registration experience
* Community nursing experience
* Experience of management of caseloads


Knowledge

* Variety of evidence based clinical skills
* Confidence to teach student nurses, trainee nursing associates, patients, carers and members of the multidisciplinary team
* Understanding of evidence-based nursing, multidisciplinary working and clinical governance
* Competence in male and suprapubic catheterisation
* Venepuncture and IV therapy
* Cannulation
* Management of syringe drivers


Skills & Abilities

* Ability to communicate effectively and clearly in English in both written and verbal formats
* Clear and accurate documentation skills
* Ability to understand professional and ethical issues
* Ability to reflect on and critically appraise own performance
