Newburn deliver clinical care to circa 45,000 patients across three practice sites with a population demographic covering Aberdeen city centre, Northfield, a significant population of Aberdeen University students and the Fyvie/Oldmeldrum population in Aberdeenshire. This allows for an interesting and varied clinical workload.

We are also passionate about trying new approaches to deliver Primary Care and have a diverse consulting team comprising of GPS, ANPS, Practice Nurses, Treatment Room Nurses, CTAC Health Care Assistants, Physiotherapists, Mental Health Nurses and a Pharmacy Team. We are also a teaching and GP training Practice with FY2s and GPSTs regularly rotating through the Practice.
This post is suited to an established Practice Nurse with existing chronic disease management experience to join a friendly team to work primarily within the nursing team and available to work across all 3 of our sites based at Carden, Old Aberdeen and Fyvie/Oldmeldrum and any other location under our remit. Applicants must be fully competent across all treatment room duties.
Key Responsibilities
* Deliver and manage chronic disease clinics, including diabetes and/ or respiratory reviews.
* Provide individualised care planning, lifestyle advice, and self‑management support.
* Undertake assessments and ongoing monitoring in line with clinical guidelines and local care pathways.
* Deliver comprehensive treatment room services, including wound care, injections, ECGs, and cervical screening.
* Maintain accurate and timely clinical records using practice systems.
* Participate in audit, clinical governance, and quality improvement activities.
* Work collaboratively with GPs, ANPs, pharmacists, and the wider multidisciplinary team.
About You
* Experience working in primary care desirable.
* Proven experience in chronic disease management, specifically diabetes and/or respiratory care.
* Fully competent in all treatment room areas.
* Able to work independently, prioritise workload, and make evidence‑based clinical decisions.
* Excellent communication skills with a compassionate, patient‑centred approach.
* A positive team member who contributes to a supportive working environment.
* Valid NMC registration
* Diabetic Diploma or equivalent or Respiratory Diploma in COPD plus Asthma or equivalent
* Deliver comprehensive treatment room services, including wound care, injections, ECGs and cervical screening
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a multidisciplinary team
* Commitment to patient safety and confidentiality
* Spirometry competence
* Contraception review experience
* Nurse Independent Prescribing Qualification
* Experience with EMIS or similar clinical systems
* Familiarity with NICE guidance and local care pathways
