Anticoagulant Monitoring Nurse or Pharmacy Technician - North East

To provide efficient andsafe near patient testing and monitoring of patients disease state andanticoagulation status.

To provide specialistadvice and support to patients, relatives, and other health care professionalsinvolved in the patients care.

To provide the therapeuticadjustments of oral anticoagulants within the Anticoagulant Protocol to ensurepatients are kept targeting INR.

To perform doseadjustment and titration, by following the Anticoagulant Protocol.

To ensure maintenance ofanticoagulation kit and equipment.

Work as part of the Anticoagulantteam, in clinics and home visits


Main duties of the job

* Promote and monitor the appropriate and safe use of anticoagulanttherapies within the service under the direction of the Contract Lead.
* Works under direction of the Anticoagulant Protocol.
* Performs near patient testing of all anticoagulant patients in aclinic setting.
* Receive, interpret and dose patients accordingly to maintain theirtarget INR, considering interacting medication, disease state and any otherfactors.
* Initiate anticoagulant patients and manage transfer of their carefrom a hospital setting.
* Administer vitamin K in-line with Patient Group Directions.
* Provide advice and support to patients and their relatives andcarers as appropriate.
* Maintain anticoagulant kit and equipment, ensuring adequatesupplies to carry out the clinic and domiciliary visits.
* Participation in NEQAS scheme and other routine equipmentvalidation.
* Work as part of the anticoagulant team and wider team of healthcare professionals, fielding queries and offering advice.

Job responsibilities

Person Specification


Experience

* Previous anticoagulant clinic experience.
* Previous experience in a Peripatetic role


Qualifications

* Registered with NMC
* Evidence of CPD and commitment to life-long learning
* Extended/independent nurse prescriber qualification
* Work calmly under pressure and to deadlines
* Flexible and proactive attitude
* Enthusiasm for the role
* Flexible approach to work, in line with the needs of the service such as evening or weekend working as necessary
* Able to work as part of a team
* Ability to work both as a team member and independently
* Basic IT literacy i.e. use of e-mail, the internet and microsoft office applications
* Ability to plan own workload with priorities and objectives
* Demonstrate an understanding of the data protection act and patient confidentiality, clinical and information governance and Caldicott requirements
* Knowledge of anticoagulant management in patients.
* Knowledge of SytemOne
* Knowledge of INRSstar
