Location: Oldham (Hybrid – 37 hours per week)Reporting to: Live Well ManagerContract: PermanentSalary: £27,411.70 Improving lives in Oldham First Choice Homes Oldham (FCHO) have an exciting opportunity for a Live well Advisor to play a key role in delivering an intensive housing management and wellbeing service to our customers. You will support customers to maintain their tenancies, promote independence and reduce isolation. Through tailored advice, advocacy and partnership working, you will help customers overcome barriers and improve their overall health and wellbeing. This is a rewarding, customer focused role where you can make a real difference to peoples lives. The Impact You’ll make • Undertaking holistic assessments to identify individual needs, risks and barriers• Developing tailored action plans that promote independence and tenancy sustainment• Maintaining regular contact with customers in line with agreed plans• Signpost customers to access financial advice, welfare support and budgeting guidance• Support customers to access GP services, health professionals, local authority services and community resources• Assisting with housing related matters including repairs reporting and liaison with internal and external partners• Managing your own caseload, ensuring accurate and confidential record keeping• Working within safeguarding and risk management frameworks• Promoting the Live Well Service internally and externally• Building strong relationships with partner agencies to achieve positive customer outcomes Our Ideal Candidate We’re looking for someone who genuinely cares about people and is motivated to make a difference. You will have: • Experience working with vulnerable customers or housing related issues• Experience in a customer facing role• Experience assessing needs, identifying risks and providing appropriate support• The ability to manage a caseload and prioritise effectively• Strong communication skills, both written and verbal• The confidence to advocate for customers when required• Good digital skills and experience using case management systems• A clear understanding of safeguarding, confidentiality and professional boundaries• A strong alignment to our values: We listen, We own, We care, We act, We learn, We keep it simple An Enhanced DBS check is required for this role.
