Mid Norfolk Primary Care Network (PCN) Clinical Pharmacist Role
Mid Norfolk PCN has an opportunity for an experienced clinical pharmacist, on a full-time basis, to join their growing team supporting six GP practices across the Dereham area.
Main Responsibilities:
* Manage the prescription request workflow.
* Conduct complex face-to-face medication reviews, focusing on polypharmacy, deprescribing, and opiate prescribing.
* Coordinate network quality activities such as sharing best practices and cross-network audits.
* Develop and oversee medicines optimisation services within the network practices.
* Lead quality improvement initiatives, clinical audits, and manage medicines safety and antibiotic stewardship.
* Support the management of patients with long-term conditions.
* Perform clinical medication reviews and provide prescribing and monitoring recommendations to senior pharmacists, nurses, or GPs.
* Collaborate with care home staff to enhance safety in medicine ordering and administration.

Candidate Requirements:
You should be a qualified pharmacist with significant primary care experience. Essential qualifications include:
* A minimum of 2 years (or equivalent) post-qualifying experience as a pharmacist.
* An undergraduate degree in pharmacy.
* Working towards an independent prescribing qualification.
* Excellent interpersonal, influencing, and negotiating skills.
* Strong written and verbal communication abilities.
* Ability to convey complex and sensitive information clearly to diverse patients.
* Deep therapeutic and clinical knowledge with a focus on evidence-based healthcare principles.
* Excellent IT skills.
* Capability to plan, manage, monitor, advise, and review medicine optimisation issues for long-term conditions.
* Ability to work effectively both independently and as part of a team.
Desirable qualifications include holding an independent prescribing qualification and a clinical diploma.
