Job Summary
INTERVIEWS TO BE HELD ON WED 17TH SEPTEMBER
The primary purpose is to support a Primary Care Network (PCN) and its member practices in the Pudsey / Bramley area in all aspects of medicines optimisation. The Post Holder will work within their clinical competencies as part of a multi‑disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to management of long‑term conditions, minor illness, reviewing systems for safer prescribing, actioning acute prescription requests, addressing both the public health and social care needs of patients, contributing to achievement of QOF and locally commissioned quality improvement schemes, undertaking clinical audit, and developing, promoting and implementing high quality, evidence‑based and cost‑effective use of medicines in primary care in a way that maximises benefits to patients while minimising risks (clinical, legal and financial). This list is not exhaustive and additional duties may be decided by the PCN Leadership Team and the Senior Clinical Pharmacist.

About Us
West Leeds PCN is a mature and visionary PCN boasting a workforce of approximately 50 staff across multiple clinical disciplines. We also have our own premises (non patient facing) where most of the staff are based day to day (if working remotely) which lends itself beautifully to instant access, in house MDT working. We employ: Clinical Pharmacists, Pharmacy Technicians, Dietitians, Social Prescribers, Health & Wellbeing Coaches, Care Co-ordinators, Administrative and support staff, Specialised Frailty and Home Visiting team (Advanced Clinical Practitioners), Specialist Cardiac Nurse, Physiotherapists (First Contact).
In this role your employment will be with West Leeds Primary Care Network but we will be looking to redeploy successful candidates into our member practices.
