Therapy Assistant Practitioner

This is an exciting and unique opportunity to work as a band 4 Therapy Assistant Practitioner within the Community Access Team.

We are looking for an individual who can demonstrate a person- centred approach, interested in working as part of a multidisciplinary team and likes to work in a fast paced environment.

Our service is based in the Norfolk and Norwich University Hospital. At the front door we provide an Admission Avoidance service for patients who present to the Emergency Department. At the back door we attend the acute wards to support the transfer of patients to our community hospitals under the discharge to assess pathway.

As a band 4 you will be expected to work with the multidisciplinary team to facilitate early discharge from hospital and prevent unnecessary admissions.

This is a full time 37.5 hours a week post working over 7 days including Bank Holidays. Shift patterns are 8am-8pm.

Please note that parking is not available on site.


Main duties of the job

* Work as part of the Community Access Team to support the service
* Assess patients with straightforward requirements based on predetermined department protocols.
* Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
* Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.
* Demonstrate problem solving, and contribute to the solution, working with colleagues.
* Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
* Be responsible for ensuring equipment is used safely, following appropriate training, and that it is maintained appropriately.
* Contribute to the requisitioning of supplies and equipment through electronic ordering systems.

Skills, abilities and knowledge

* Good communications skills

Qualifications

* NVQ 3, or equivalent knowledge and experience
* Foundation degree programme, or equivalent demonstrable knowledge and skills acquired through training and experience

* Experience of working in a health or social care environment
* Evidence of ongoing learning
* Experience of working in peoples homes
* Experience of working with people in the voluntary/private sector
* Experience of working with people at end of life
