Responsibilities Assess, diagnose, plan, implement, and evaluate care for patients with complex health needs within your scope of practice. Independently manage a diverse caseload, ensuring safe and effective clinical decision-making and holistic patient care. Provide highly specialized advice and support to patients, family members, and caregivers, ensuring sensitivity to cultural and personal differences. Collaborate with multidisciplinary teams to develop, implement, and evaluate care pathways and protocols. Contribute to quality improvement initiatives, including research, audits, and evidence-based practice to enhance the quality of care provided. Mentor and support junior staff and students, promoting their professional growth and development. Comply with legal, regulatory, and organizational policies that uphold patient care and safety standards. Qualifications Registered Nurse (RN) with current registration and licensure. Master's degree in Advanced Clinical Practice or equivalent qualification/training. Demonstrable experience in advanced nursing practice, preferably in a multidisciplinary healthcare setting. Excellent assessment, diagnostic, and clinical decision-making skills. Strong interpersonal and communication skills to work effectively with patients, families, and colleagues. Ability to maintain calmness and professionalism in high-pressure or emergency situations. Dedication to continuous professional development, with a commitment to staying current on industry advancements and best practices. Demonstrated commitment to advancing equity, diversity, and inclusion within healthcare settings. Day-to-day Perform advanced clinical assessments and interventions for patients with acute and chronic conditions. Lead and participate in ward rounds, patient consultations, and care planning meetings. Deliver education to patients regarding diagnoses, treatments, and health management plans. Work collaboratively with physicians, allied health professionals, and support staff to develop individualized care plans. Provide mentorship and training to nursing staff, junior ACPs, and students. Document care accurately and maintain communication with patients and families regarding care progress. Contribute to ongoing research or quality improvement projects as part of organisational initiatives. Participate in team discussions, contributing ideas to ensure an inclusive workspace for both staff and patients.
