Job Overview
This is an excellent opportunity to work as part of the specialist frailty multidisciplinary team (MDT). Our team delivers patient‑centred care with timely assessments, interventions and discharge planning for older people in an acute hospital setting.
Key Responsibilities
* Complete individual therapy assessments for older patients.
* Engage patients in rehabilitation to regain function.
* Provide support and guidance to other therapy support staff.
* Assess patients in their home environment when required, working under the supervision of qualified clinicians.
* Develop and maintain competency levels with the support of in‑house training.
* Work flexible 7‑day shifts, including weekend rota coverage.
Qualifications & Experience
* Previous experience working with older people in an acute hospital or community setting (essential).
* Full UK driving licence (preferred).
* Motivated, enthusiastic and well‑organised with effective communication skills.
