An exciting opportunity has arisen for an enthusiastic and self‑motivated individual to join the North Integrated Discharge Hub based at Royal Stoke Hospital.
The role of the Rapid Discharge Facilitator will be based in our Triage hub, providing hospital discharge guidance and support to our referring acute hospital trust.
The successful candidate will be part of a triage team that receives referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients’ needs as part of a multi‑disciplinary team with a key role in decision‑making.
Main duties of the job
The post holder will liaise with a variety of people (service users, colleagues, members of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual’s ability/potential for reablement via the discharge to assess pathways while supporting admission‑avoidance assessments to prevent a hospital admission.
The post holder will work autonomously as part of a hub-and-spoke model and will be expected to use their own initiative and professional judgement, while also receiving support from the surrounding team and through the clinical and professional networks accessible through the community teams.
Job responsibilities
The service operates a 7‑day a week service from 8am to 8pm, including weekends and bank holidays. Hours will be negotiated on a pro‑rata basis.
We are looking for a person who is passionate about making a difference to patients and enabling individuals to live their best lives through our reablement service. They should possess effective telephone and communication skills, IT skills, and exceptional organisational skills.
The post holder will receive robust personal development conversations outlining clear objectives and training needs. Regular professional and clinical supervision and opportunities to link with wider clinical networks across the Trust will also be provided.
Person Specification
* Level 3 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent, plus additional knowledge and experience of care practices and procedures to diploma level.
* Level 4 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent.
Experience
* Experience of working in a team, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual’s needs.
* Management of complex discharges and procurement of packages of care; experience in working with an integrated team.
