Support reablement by enabling early, safe discharge from hospital and preventing unnecessary care placements. Work closely with Social Workers, health professionals,carers, and families to coordinate care. Act as an advanced practitioner within OT and across Tricuros services, offering expertise in postural seating, positioning, complex moving and handling, cognition, and contracture management. Assess and manage risks in partnership with clients, their families/carers, and other professionals involved in their care. What you need Current registration with the Health and Care Professions Council (HCPC)as an Occupational Therapist. Professional qualification in Occupational Therapy(Diploma or Degree). Experience of working with adults in a health or social caresetting. Strong clinical reasoningand problem-solving skills. Experience of multi-professional and multi-agency working. Competent in using ICT systemsfor clinical recording. Excellent communicationand team-working skills. Valid UK driving licenceand use of own vehicle.
