Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes.

The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England's website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS.

The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull. Our workstreams include:-

o Provision of comprehensive Geriatric Assessments from both within the Integrated Care Centre and patients usual place of residence including Care Homes located across Hull and East Riding.

o Medical support to several community bedded units, used as a step up/down facility to support hospital admission prevents and the discharge to assess model (D2A)

o Advice and Guidance line accessible to Healthcare Professionals involved in the care of a frail patient including Care home staff o Urgent Community 2 hour Response Service


Main duties of the job

The successful candidate will be qualified in NVQ level 3 Health and Social Care or equivalent. Also essential are the skills and competencies (or the willingness to work towards, for which full training will be given) to undertake basic clinical observations and delegated tasks including implementing treatment plans under direction from a registered member of the team

The abilities to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, as is working experience/knowledge of local pathways and referral procedures.

We can offer flexibility to the successful candidate in terms of either full or part-time working. The service is open 365days per week with expectation for successful candidates to be included on weekend and bank holiday rotas.

Person Specification


Qualifications

* Formal qualification in relevant subject e.g. Level 3 qualification / Basic or Advanced OR relevant in-service training / education
* English language GCSE or equivalent; Mathematics GCSE or equivalent
* Completed Care Certificate


Knowledge

* Awareness of current health issues
* A good understanding of the role of Support Worker in a clinical setting
* Knowledge of Safeguarding
* Knowledge of Information Governance
* Awareness of what is involved in working in a Multi-Disciplinary Team
* Awareness of what is meant by integrated Care


Experience

* Experience of working in healthcare, preferably NHS or related setting such as Social Care
* Experience working as part of a team
* Previous post as a Support Worker, Health Care Assistant or Carer.
* Experience of supporting junior members of a team
* Experience of working with the elderly person
* Experience of using a Patient system/database
* Experienced in use of using SystmOne
