Your Role
We are looking for a Social Care Officer to join our Adult Autism Team.
You will assess autistic adults over the age of 18 under The Care Act, taking a strengths based approach to support them to identify their outcomes.
Working as part of a multi-disciplinary team, you will support planning, monitoring and reviewing (face to face and via telephone) and work with service users and / or their carers to ensure that vulnerable people are safeguarded and supported in managing risk. You will carry our annual reviews, take part in a duty rota.
You will undertake training and take responsibility for your self-development, identifying your learning and development needs. In addition there will be the potential for you to apply to undertake the Social Work degree Apprenticeship. You would need to have Maths and English at GSCE Grade C or be willing to work towards this before you qualify as a Social Worker.

We are looking for an enthusiastic and passionate Social Care Officer who wants to make a difference to adults with a learning disability. We are looking for someone who can work collaboratively, think creatively and has a good level of communication and written skills.
