Your role:
The role is a rewarding position within the team that supports residents of the borough back into their own homes.
As a Reablement Support Worker, you will be part of the team providing integrated, multidisciplinary programmes of Reablement across the borough. We are part of Urgent Care, which includes Reablement, Therapists, Social Care Workers, Housing, and Health professionals.
The Reablement service operates 7 days a week, 365 days a year, including Bank Holidays, between 07:00 and 22:00. This role may require hours beyond the normal schedule based on service needs.
You will work every other weekend from 08:00 to 22:00.
About you:
You must be creative, self-motivated, and have a strong customer care ethic. Good interpersonal and communication skills are essential, along with the ability to promote independence.
You will support residents in their homes, using an asset-based approach to promote independence, helping people remain at home and connect with their communities to improve well-being.
A valid driving licence and the ability to travel as required are necessary.
This position requires an enhanced DBS check.
