Ealing Short Term Rehabilitation – Band 7 Community Occupational Therapist

We have exciting opportunities for a Permanent Band 7 Occupational therapist to join our specialist multidisciplinary community rehabilitation service.
Please note that our service has 2 bases and interviews will be held in person at our office in Level 10 South Ealing Hospital, London or EDTC, Southall UB1 2SH.
Main Duties
The Ealing Short Term Rehabilitation (STR) team provides therapy‑led rehabilitation to help patients improve their abilities and achieve their rehabilitation goals, through therapy led care. We focus on achieving goals and enabling people to regain independence and social integration effectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistants offer support for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way. Our team includes Physiotherapists, Occupational Therapists and Rehabilitation Assistants. The team provide a service in a variety of community locations but mostly in the patient's own home.
The post holder will contribute to the occupational therapy provision within the STR team and will deliver skilled OT care to patients in their own home, based on a thorough holistic assessment of their clinical and personal needs and the agreement of an individualised rehabilitation care plan to the standards set by RCOT and HCPC.
The post holder will participate in service‑wide seven day working on a rota basis.

Detailed Job Description And Main Responsibilities
* To undertake highly specialist assessment of patients, interpreting complex findings, to enable an appropriate treatment plan to be formulated, delivered and progressed for a wide range of complex conditions, using highly developed clinical reasoning and skilful therapeutic handling developed from specialist knowledge and using extensive practical experience to guide the process.
* To assess, prescribe, issue, and instruct patients in the safe use of a variety of equipment including a range of functional aids, manual handling aids and mobility equipment.
* To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs, and to be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate. To evaluate and develop treatment programmes.
* To keep accurate up‑to‑date records on patient care and equipment provision, which comply with quality standards of Trust/Professional/Service policies, meet legal requirements and that can be shared with colleagues and carers when appropriate.
* To advise and teach patients, carers and other professionals to participate in occupational therapy and intervention plans for the individuals.
* To ensure a high standard of clinical and professional practice.
* To support, supervise and complete appraisals for junior Occupational Therapists, Rehabilitation Assistants, students on placement and locum therapists.

Person specification
Education/Qualifications
Essential criteria:
* Degree/ Diploma Occupational therapy
* Registration with HCPC as an Occupational Therapist
* Evidence of continued professional development
* Relevant professional body membership
Desirable criteria:
* Post graduate qualification relevant to the healthcare environment
Experience
Essential criteria:
* Extensive post‑qualification experience across core areas within NHS or similar healthcare economy
* Substantial experience in a rehabilitation setting
* Experience in managing complex individual & team’s caseload of patients
* Experience of supervising and appraisals of junior colleagues & students
* Experience of performance management & dealing with complaints
Desirable criteria:
* Previous community therapist experience in NHS or similar healthcare economy
* Experience of case management including multi‑agency working
* Experience of leading a team
Ability and Skills

* Contribute towards improving quality in own work area and encourage others to do so

* Ability to travel across the Trust to fulfil the requirements of the post (Valid UK Driving licence/ Car owner/ able to use public transport to manage clinical expectations of caseload)

* Knowledge of developments within NHS and specialist occupational therapy professional knowledge
Desirable criteria:
* Knowledge of long‑term conditions including end of life care and the drive toward care for these patients
* Knowledge and understanding of the NHS modernisation agenda and the implications for community services
