Locum Band 8a Pharmacist in the Community Frailty Team – Plymouth, UK
Are you ready to embark on an extraordinary journey as a Band 8a Pharmacist within our Community Frailty Team in Plymouth? This ongoing locum role offers £36 per hour. Join us on a full-time basis and make a real difference in the lives of those in your community.
What you will do
* Clinically screen medication charts on the ward
* Screen discharge prescriptions and one-stop orders
* Complete medicines reconciliation
* Handle ward MI queries and work in MDT settings
* Conduct ward CD audits and order medicines
* Collaborate closely with Medicines Optimisation technicians
* Interpret blood results in relation to medicines
Responsibilities
As a locum pharmacist, you will provide clinical screening, medicines management, and collaborative care within the Community Frailty Team to support optimal patient outcomes.
Requirements
* Registered pharmacist with the GPhC; hospital experience is ideal but not essential if you are qualified and have been working for over a year
* Interest or experience in frailty, stroke, brain injuries, or rehab is preferred
* Knowledge of polypharmacy and deprescribing is advantageous
* HCPC registration required
* Full driving licence is necessary for travelling between sites
