Overview
The post holder is an experienced Practice Nurse, responsible for delivering safe and effective patient care to the practice population using autonomous clinical skills and a broad theoretical knowledge base. They will manage a diverse clinical caseload in a primary care setting, contribute to developing nursing services, work independently, and support audit, research, and the implementation and evaluation of local standards, policies, guidelines and care pathways. NMC requirements will be met, and the individual will work within the NMC Code of Conduct to agreed practice standards and protocols.
Main duties
* Accountable for their own practice and capable of independently managing their clinical workload, responding to patient needs and ensuring patient choice and access to services.
* Mentor and support other nurses and staff in developing and maintaining clinical skills.
* Participate in clinical supervision.
* Be aware of budgetary constraints and assist with cost‑effective decisions.
* Use referral pathways for nurses as appropriate.
* Provide input on evidence‑based practice and care interventions within their scope of knowledge.
* Occasional Saturday working may be required.

Job Description
Role: Practice Nurse for Maternity Cover December 2025 – End of December 2026
Hours of Employment: 12 hours
Responsible to: Practice Manager; Accountable to GP Partners & Nurse Manager
Primary Duties & Areas of Responsibility
* Clinical Role: Assess, plan, develop, implement and evaluate treatment programmes to promote health and well‑being.
* Develop and deliver specialist nurse‑led services, including chronic disease management; maintain disease registers; conduct annual patient self‑management reviews.
* Make autonomous clinical decisions, provide safe, evidence‑based, cost‑effective care.
* Evaluate patient test results with GP as needed and initiate appropriate treatment.
* Refer to other primary, community and secondary care teams as appropriate.
* Conduct regular medication reviews for suitable patient groups.
* Provide travel health advice and vaccination services; manage vaccines and stock safety.
* Perform procedures (injections, venepuncture, BP, urinalysis, etc.); support clinicians with chaperoning as required.
* Promote health education on diet, contraception, smoking cessation, exercise, etc.; initiate health screening programs.
* Support and empower individuals, families and groups to improve health and well‑being.
* Contribute to GMS contract targets and participate in clinical supervision.
* Maintain accurate records per NMC rules and data protection requirements; work with the MDT.
* Engage in ongoing training and professional development; teach and mentor practice staff and students.
* Promote specialist nursing expertise within the practice and with the public.
Professional & Managerial Roles
* Attend internal and external meetings; maintain a personal training plan and seek relevant development opportunities.
* Promote evidence‑based practice; monitor clinical practice through quality assurance; maintain professional registration and NMC Code compliance.
* Record consultations accurately; stay informed on health policy; collaborate with colleagues internally and externally; demonstrate leadership.
* Promote learning and use of up‑to‑date technology in practice.
* Participate in protocol development; delegate tasks to junior nursing staff; support cost‑effective resource use.
Working Conditions
* Exposure to body fluids, blood, wounds; occasional exposure to aggressive behaviours.
* Travel between sites and clinic delivery at branch surgeries; home visits may be required (mileage reimbursed).
