We have an exciting opportunity for a project manager to join an evolving Integrated Front Door team. We are seeking motivated an enthusiastic individual to lead on the development of ongoing collaboration of services within our Integrated Front Door service. NHSWI is developing an Integrated Front Door Service bringing together the workforce across the Acute Assessment Unit, Hospital at Home and the Urgent and Unscheduled Care with close links to the Emergency Department staff. The common aim is to enable patients to be cared for at home, reducing unnecessary in-patient hospital admission thereby enhancing patient care and clinical outcomes. The post holder will lead key projects within the Integrated Front Door Service to achieve elements of the current Operational Plan. The post holder will review current systems, pathways and models using data to best inform decision making with respective departments and teams. The post holder will be accountable to the Service Manager for Urgent and Unscheduled Care and be expected to coordinate and complete projects within timelines, scope and allocated budget. Applicants should have good communication skills, and a commitment to developing change and promote innovation within the front door services. The ability, drive and commitment to work as part of a team, sometimes at a distance, and to contribute to the local development of primary care and acute services is important.
