Join our dynamic ADAPT Team, a frontline service based at the Midland Metropolitan University Hospital (formerly City Hospital), supporting citizens from Birmingham’s most deprived communities.
About the ADAPT Service
The ADAPT (Admission Avoidance and Discharge Planning Team) is a front-door, integrated health and social care service operating 7 days a week. We work closely with Emergency Departments and medical assessment units to prevent unnecessary hospital admissions and support timely, safe discharges.
Our team plays a vital role in:
* Supporting patient flow across the hospital system
* Identifying community-based solutions
* Signposting and advising citizens and carers
* Conducting Care Act and Mental Capacity assessments
The team helps improve health outcomes and experiences for citizens from areas such as Aston, Lozells, Newtown, Handsworth, Ladywood, Perry Barr, Summerhill Terrace, and HM Prisons.
What We’re Looking For
We’re seeking Qualified Social Workers who:
* Hold a CQSW, DipSW, CSS or equivalent Social Work England‑recognised qualification
* Registered with Social Work England
* Have experience working with vulnerable adults (hospital experience is desirable but not essential)
* Can work effectively in a fast‑paced, multidisciplinary environment
* Is confident in making decisions under pressure and managing complex needs
* Is committed to equality, continual professional development, and service improvement
