Job Description
We are currently seeking an Occupational Therapist to join our Adult Social Care team in Thatcham. As an Occupational Therapist, you will play a vital role in assisting Service Users, Carers, and various partners in optimising reablement services, equipment usage, and living environment adjustments to enhance individual independence and achieve desired outcomes.
Key Responsibilities:
* Collaborate with Service Users, Carers, and Social Care Practitioners to facilitate Self Directed Support Planning and Personal Budget setup.
* Maximise opportunities for vulnerable individuals to maintain independence by assessing and managing risks effectively.
* Coordinate with reablement services, Health colleagues, and other stakeholders to develop individual Self Directed Plans.
* Ensure compliance with eligibility criteria and best value principles in resource allocation.
* Provide Occupational Therapy expertise to support colleagues and enhance service delivery.
Main Duties:
* Adhere to professional standards set by the College of Occupational Therapy.
* Advise and prescribe reablement programs tailored to individual needs, goals, and safety.
* Support service users in constructing Support Plans and accessing appropriate resources.
* Process applications for Disabled Facility Grants and ensure progress monitoring.
* Implement safeguarding procedures to ensure service user safety.
* Collaborate with stakeholders to manage risks effectively and promote independent living.
* Promote equality, fairness, and dignity in service delivery.
* Maintain accurate records and reports.
* Engage in self-development through supervision, appraisal, and training opportunities.
Person Specification:
Essential:
* Professional qualification in Occupational Therapy and registration with the Health Professions Council.
* Experience in Adult Social Care Sector.
* Knowledge of national legislation including Care Act and Mental Capacity Act.
* Familiarity with Safeguarding Adults procedures.
* Understanding of self-directed support process, personalisation, and personal budgets.
* Ability to use Outlook, web browsers, and Microsoft Office tools.
* Excellent communication and interpersonal skills.
* Commitment to anti-discriminatory and anti-oppressive practice.
* Positive attitude towards risk enablement and management.
* Public-facing role requiring proficiency in English communication.
* Enhanced DBS check with relevant barred list/s.
* Willingness to participate in Duty Rotas and possess a full driving license.
* Ability to provide training on techniques related to individual conditions.
Desirable:
* Moving and Handling Assessor qualification.
* CPD record.
* Familiarity with services available in the community and health services.
