We are looking for a passionate and skilled Band 6 Community Nurse Team Lead to join our Thanet Community Nursing Team, soon to be based at our brand new £10million state-of-the-art Thanet Health Hub.
Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.
This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. And here at Kent Community Health, we're right at the forefront.
Planning to open in Autumn 2025, our new Thanet Health Hub will consist of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK.

Main duties of the job
You will play a vital role in our multidisciplinary services; managing patients with complex needs in the community. You’ll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, with access to great supervision and a clinical lead to make sure you are on the right track.
You will be supporting junior colleagues with clinical development, helping them to reach their potential and develop the workforce of the future. If you are applying for a Community nursing role, it is essential that you hold the Specialist Practitioner (District Nursing) qualification. We will consider development and secondment opportunities.

Qualifications
Essential criteria
1. Registered health care professional (NMC registration).
2. Health related degree and evidence of relevant post graduate development at Level 6.
3. Evidence of Specialist qualifications relevant to area of practice (examples specialist practitioner district nursing, non-medical prescribing; mentorship, coaching, clinical expertise).
4. Full UK Driving Licence
Desirable criteria
5. PTLLS/DTLLS, PGCE or Certificate in Education.
6. Management or supervisory qualification or experience of study.
7. Working towards Master level study
Experience
Essential criteria
8. Minimum of 5 years’ experience in area of practice.
9. Practical experience in managing health related projects/complex caseloads
10. Supporting others (mentor / educator / supervisor) experience.
Desirable criteria
11. Experience of leading and managing teams.
12. Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ CIS).
