Overview
An exciting opportunity for a full time First Contact Mental Health Practitioner with a specialist interest in working with young people, to work within primary care within the South Hambleton and Ryedale Primary Care Network (SHaR PCN).
This role is fundamental to the development of inclusive and integrated mental health services. The post holder will fulfil a 'first contact' specialist mental health role within GP Practices across the Primary Care Network, seeing patients who would otherwise have traditionally seen a GP. The role sits within GP Practices and will facilitate the interface between Primary Care and Secondary Mental Health Services to provide specialist advice and support, assessment, treatment, education and solution-focused approaches to the local Primary Care Teams, for patients and carers.
The post holder will work in a collaborative, multi-agency environment and contribute to safe and effective day-to-day operation of the PCN and the timely management of clinical enquiries and decision-making.
Travel across the locality and access to a suitable vehicle for business purposes is essential. Adjustments can be considered in accordance with the Equality Act 2010 where needed.
Responsibilities
* Provide initial assessment and signposting to appropriate services, or deliver time-limited psycho-social interventions directly in GP surgeries, patients’ homes and other primary care settings.
* Offer clinical advice to GPs, including specialist opinion, and act as a specialist advisor to GP practices with a pathway management function.
* Manage daily clinics and support short-term interventions (average duration around 6 weeks) for patients with mild to moderate mental health difficulties.
* Develop and maintain partnerships and networks with key stakeholders across the PCN and wider mental health care system; advise or obtain advice for ongoing patient care management.
* Foster a culture of partnership between Primary Care Teams and Mental Health Services with clear communication and access to specialist knowledge and advice.
* Facilitate joint working with PCN, Secondary Care providers and Voluntary Care Sector to expand available services and interventions for communities served.
* Provide clinical advice, consultation, and support to care providers within primary, secondary, voluntary and statutory services as required.
* Collate and organise contemporaneous information about local opportunities and resources (e.g., voluntary and community organisations, educational and recreational opportunities) for patients referred to the service.
* Facilitate signposting to appropriate services and collaborate with care coordination teams and social prescribing link workers where applicable.
* Work with the PCN, GP practices and Social Prescribers/practice-based mental health workers regarding adult mental health needs at local population level.
* Ensure pathways enable smooth transition between primary care and secondary mental health services in line with community transformation principles.
Qualifications and Requirements
* Registrations welcomed: NMC Mental Health Nurses, Social Workers (England), HCPC Occupational Therapists are encouraged to apply.
* Ability to travel independently across the locality and access to a suitable vehicle for business purposes; suitable adjustments can be made under the Equality Act 2010 where necessary.
