Your new role
1. Support service users from entry into the service through their treatment and recovery journey.
2. Deliver high-quality care to reduce drug and alcohol-related harm.
3. Promote involvement of carers, service users, and themunity.
4. Provide advocacy for access to partnership services.
Core Duties
5. Assessment & Planning
6. Conduct screening, triage, and risk assessments.
7. Develop recovery plans tailored to individual needs.
8. Assess suitability for group work and onwards referrals.
9. To deliver outreach services to key sites and groups related to substance misuse provision, homelessness, promoting access for service users and concerned others.
10. To adhere to CGL policies and processes in delivery of services – particularly lone working and health and safety related to working withplex service users.
11. To undertake focused work withmunities of interest, including but not exclusive to, sex workers, homeless people, BAME groups and those with mental health issues.
12. To provide rapid and flexible interventions where required to meet the needs of the service users.
Interventions
13. Deliver psychosocial interventions (, CBT-based approaches).
14. Provide harm minimisation and brief interventions.
15. Facilitate 1-to-1 key work sessions, group work, and structured programmes.
Recovery Support
16. Help individuals build recovery resources and access peer/mutual aid groups.
17. Support social reintegration and resilience building.
18. Promote recoverymunities and links with local support networks.
Collaboration
19. Work with prescribing clinicians to support substitute prescribing regimes.
20. Liaise with multi-agency partners (healthcare, housing, probation, etc.).
21. Advocate for service users to access wider health and social care services.
Safeguarding &pliance
22. Ensure confidentiality and consent protocols are followed.
23. Adhere to safeguarding practices for children and vulnerable adults.
24. Maintain accurate records and reporting.
