Permanent, full time, 37 hours per week. This vacancy is for a social worker within the autism pathway and is intended specifically to support autistic customers.

Exciting opportunities have become available for social workers, due to the ongoing development and expansion of Learning Disability and Autism services in Rotherham Adult Social Care. Rotherham is on an exciting journey to transform its Adult Social Care service to ensure every adult is supported to maximise their independence and be safe, healthy and well.

About the Team
The Learning Disability and Autism team aims to ensure that every person with a learning disability/autism has access to support, services and opportunities. We work with adults of all ages and cover the whole Borough with bases at Riverside and Maltby offices. We work with professionals, families and carers to keep the person at the centre of the support we offer. Every day is different and the breadth of work spans duty interventions, assessment and care management, safeguarding adults, and Court of Protection work.
We are transforming our LD/Autism services and have expanded the number of people we are able to support. We work holistically with agencies to deliver integrated support.
Responsibilities
* Co-create tailored care and support plans that empower autonomy and independence.
* Collaborate with families, carers, health, housing and voluntary sector partners to deliver holistic, integrated support.
* Monitor and review plans regularly to ensure they evolve with the individual's changing needs.
* Advocate for service users, championing their rights and choices.
* Maintain clear records and prepare professional reports for case conferences and reviews.
Essential Qualifications & Experience
* Qualified (BA/MA/DIPSW) with valid Social Work England registration.
* Proven experience in adult social work, particularly supporting autistic adults and those with learning disabilities.
* Excellent knowledge of relevant legislation, including Care Act, Mental Capacity Act, Mental Health Act, and safeguarding frameworks.
* Strong assessment, analytical, planning and risk management skills.
* Confident communicator with ability to build relationships across agencies.
* IT literacy and case management system experience.
