Job overview

The Integrated Cardio-Respiratory Service is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings.

The service is a consultant led multidisciplinary team, providing a range of services including: diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, Pulmonary Rehabilitation, dyspneoa management and primary care education and support.

We are looking for a motivated and enthusiastic individual to work cooperatively as part of the multi-disciplinary team supporting colleagues in looking after patients and promoting respiratory health within ICHNT and the surrounding community. The role will entail supporting the home oxygen service as well as the new dedicated social prescribing pathway.

We are seeking an experienced professional with excellent communication skills and a passion for improving people's access to health enhancing lifestyle choices.

Main duties of the job

1. To support a multi-disciplinary specialist team in delivering delegated care to patients within respiratory.
2. Assist in the provision of care to oxygen patients, being supported and supervised by a registered clinician.
3. To provide aspects of personal healthcare as directed and delegate by the multi-professional team.
4. To support the provision and delivery of health care and health promotion advice in order that patients are informed and empowered to make appropriate health choices to support their independence.
5. To support the social prescriber in coproducing a social prescription with individuals referred and identified by the service, which provides the individual with positive health and wellbeing outcomes through addressing non-medical/social issues in their life, such as housing, fuel poverty or isolation.
6. Carry out assigned duties which maintain hygiene, orderliness and safety of the ward/department environment.

Education/ Qualifications

Essential criteria

7. GCSE grade C or above in Maths and English or equivalent experience
8. HEE Social Prescribing - Learning Link Workers
9. Home Oxygen eLearning (air Liquide)

Desirable criteria

10. Medical terminology

Experience

Essential criteria

11. Able to demonstrate an understanding of issues relating to confidentiality
12. Competent use of IT systems and ability to learn
13. Ability to work in fast paced work environment
14. Attention to detail

Desirable criteria

15. Previous experience in an NHS institution
16. Previous oxygen experience
17. Previous social prescribing experience within a respiratory service

Skills/knowledge/abilities

Essential criteria

18. Clear & effective communication skills. Organisational skills/ability to learn

19. Service sector work (preferably hospital).
20. Cerner and/or SystmOne
21. CDL
