K&W South PCN is seeking to recruit a full time (37.5 hours a week) first contact physiotherapist (FCP) to work as an independent MSK practitioner, across two GP Practices - Willesden Green Surgery (in Willesden Green) and Lonsdale Medical Centre (in Queens Park). We are looking for an applicant who can commit to working full-time hours (37.5 hours per week), across the two practices.
There are training opportunities as both practices provide MSK joint injections. Willesden Green Surgery also has a GP with a special interest in MSK medicine who provides on-site ultrasound guided joint injections with a GE Logiq-E ultrasound machine. Mentorship and support can be provided for those looking to up-skill and provide landmark based MSK injections, ultrasound guided MSK injections or become a non-medical prescriber. To support this, protected time of one full session (around 4hrs a week) will be provided for your development, equivalent to 27 days a year
We welcome UK applicants who are fully HCPC and CSP registered physiotherapists. It is preferred to have at least 3 years relevant post-graduate NHS musculoskeletal experience.

Job Role
1. Be the first point of clinical contact for patients presenting with MSK conditions or symptoms, providing the patient with the primary assessment, diagnosis and management options for their condition
2. Work independently, without day to day supervision, to assess, diagnose, triage, and manage patients, taking responsibility for prioritising and managing a caseload of patients.
3. Use advanced clinical practice skills, examination and clinical reasoning to provide informed diagnoses and treatment for patients who present with complex musculoskeletal conditions, multiple pathologies, chronic pain or pain management needs
4. Be the link between primary, community and acute services, when the patient is managed via primary care ensuring an integrated care pathway for individual patients that meets their needs. This will include provision of triage to other services and/or directly providing management and advice
5. Develop integrated and tailored care programmes in partnership with patients, providing a range of first line treatment options including self-management, referral to rehabilitation focussed services and social prescribing
6. Designing and implementing plans that facilitate behavioural change, optimise patients' physical activity and mobility, support fulfilment of personal goals and independence, and reduce the need for pharmacological interventions
7. Make use of their full scope of practice, developing skills relating to independent prescribing, injection therapy and investigation to make professional judgements and decisions in unpredictable situations, including when provided with incomplete or contradictory information. Take responsibility for making and justifying these decisions
8. Maintain relationships with MDTs, orthopaedic and community physiotherapy consultant teams in the community and acute settings.
9. Encourage collaborative working across the health economy and be a key contributor to the primary care networks providing leadership and support on MSK clinical and service development across the network
10. Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training
11. Work towards advanced clinical practitioner status
Clinical
1. Request and progress investigations (such as x-rays and blood tests) and referrals to facilitate the diagnosis and choice of treatment regime including, considering the limitations of these investigations, interpret and act on results and feedback to aid patients' diagnoses and management plans and
2. Carry own caseload as an autonomous practitioner, providing direct clinical/ physiotherapeutic care for patients with a range of complex musculoskeletal conditions and who may have significant other primary or secondary conditions/multi-pathologies
3. Be able to decide when appropriate to use advanced clinical practice skills that may include:
a. joint/soft tissue injection therapy, including administration of prescription only medication [POM] to aid treatment.
b. joint aspirations
c. non-medical prescribing
1. Ensuring physiotherapy interventions are integrated and supportive of the whole treatment aims promoting a holistic approach to condition management
2. Be able to clinically justify referral onto appropriate MSK pathways within the community or wider healthcare services. Including referral to the appropriate stage of the pathway and the use of social prescribing
3. Be able to justify clinically referrals to other specialist services. This will include referral to acute and specialist services such as surgical orthopaedics, rheumatology and neurology, other AHP services, nursing services and to the primary care medical teams
4. Recognise RED flags, serious pathology and potential underlying non-MSK related disease, that may present as MSK symptoms and to refer appropriately. When needed accelerate the patient's referral to other health professionals/services
5. Work in partnership with the patient at all times to attain maximum participation in treatment programmes. This will include working with patients from diverse social background and cultures and understanding how this will affect treatment proposals and models
6. Be accountable for own decisions and actions via HCPC registration and professional standards
7. Write contemporaneous clinical notes using EMIS, Arden's Templates and provide advice and support with additional electronic software (e.g. PhysioTec, AccuRx etc) in line with professional standards.
8. To provide sick notes when appropriate.
Professional Leadership
1. Provide expert advice and act as source of expertise in the management of musculoskeletal conditions and provide a specialist advisory service to patients, specialist physiotherapists other specialist healthcare professionals and members of the primary care team, including advising GPs on management of MSK conditions
2. Contribute to the PCN to promote the contribution of FCPs to MSK services in primary care and to the wider related services in the health economy
3. Work with the primary care team, public health professionals and other agencies to introduce a strand of service provision that highlights preventative strategies for local populations, patient groups and individuals
4. Where service improvement is required in order to achieve standards and KPIs be a lead contributor to identifying where services need to be changed
5. Take forward operational change within primary care MSK services to deliver improvements in service standards.
6. Participate in the investigation into complaints and clinical errors/incidents when appropriate.
Education and Evaluation
1. Provide and receive clinical supervision and be an active provider of mentoring and coaching of specialist clinicians from a range of disciplines
2. Maintain a professional portfolio of knowledge and skills, at advanced clinical practice level, through participation in internal and external development opportunities
3. Lead or participate in research and MDT research projects as appropriate. This may include participation in cross organisational research and audit programmes
4. Present and disseminate research and clinical audit findings across the PCN and wider service providers as required. Provide analysis of local and national data sets to illustrate service improvements and to promote service change
5. Maintain and up-date audit and data derived from a range of data sets in order provide statistical reports on outcome measures and achievement of KPIs for a range of audiences and use audit and research to develop and improve service guidelines, care protocols, delivery of triage services and referral pathways
Communication/Building Networks
1. Communicating and influence the benefits of the FCP role in primary care across the PCN
2. Promote and explain the FCP role and what it can deliver to a range of audiences including patient groups, patient population, individual patients and other primary care providers and linked services
3. Communicate effectively and appropriately with patients and carers, including:
a. explaining the diagnosis, prognosis and treatment choices available to manage multi-pathology and complex conditions
b. communicating limitations on treatment outcomes and managing expectations of patients with chronic or life limiting conditions
Key requirements
· BSc/Pre-reg MSc degree in Physiotherapy
· Health & Care Professions Council (HCPC) and CSP registration (required)
· Evidence of verification against the MSK primary care roadmap
· Current DBS check
· Desirable to have minimum of 3 years' experience in an Musculoskeletal role (Minimum NHS Grade Band 6)
