Memory Service Practitioner – work under supervision of the Memory Service Clinical Nurse Specialist, providing advice and support to clients experiencing suspected dementia or mild cognitive impairment and their carers.
Responsibilities
* Take responsibility for a clinical caseload providing care and treatment to clients referred into the Memory Service.
* Foster robust working relationships with all partners, particularly Local Authority and Primary Care.
* Contribute to the continuing improvement and development of dementia care services.
* Promote physical and mental health wellbeing, emphasizing living well with dementia.
* Act as an advocate for clients living with dementia and their carers, signposting to statutory and voluntary services as necessary.
* Complete assessments and follow‑up reviews for people within the Memory Service.
Qualifications
* Welsh language skills essential: speaking and listening Level 3 (equivalent to CEFR B2) or above; basic conversations with patients about their everyday health.
