What is the job role?We are recruiting two Community Case Workers to join our Adult Social Care team in a peripatetic role. This is a great opportunity for motivated individuals who enjoy working across a variety of settings and delivering person-centred support.You will assess the needs of service users and carers, develop and review outcome-focused care plans, and promote independence. Working closely with colleagues and partner agencies, you'll help ensure services are safe, effective, and meet agreed needs. The role also involves risk assessment, managing challenging situations, and maintaining accurate digital records.All candidates must have the Right to Work in the UK.
