Role Description
This is a full‑time hybrid role for a Social Worker, based in Crewe, with opportunities for remote work. Responsibilities include assessing the needs and circumstances of individuals and families, devising care and support plans, providing advice and interventions, and collaborating with multidisciplinary teams. The role also involves safeguarding vulnerable individuals, maintaining accurate records, and adhering to professional policies and legal frameworks. The Social Worker will act as an advocate for clients, ensuring their voices are heard and their rights are upheld.
Qualifications
* Strong skills in assessment, care planning, and case management
* Knowledge of safeguarding issues, social care policies, and relevant legislation
* Effective communication, active listening, and emotional resilience
* Experience working collaboratively with multidisciplinary teams and external agencies
* Proficiency in maintaining accurate records and utilizing case management systems
* Degree in Social Work and registration with Social Work England
