Overview
The post-holder will be a qualified nurse responsible for the delivery of practice nursing services to the practice population. Supported by the Lead Nurse, they will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will focus on health promotion, monitoring and management of long-term conditions, prevention and screening activities, immunisations and may include cervical screening. The post-holder will work collaboratively with the multidisciplinary general practice team to meet patient needs and support the delivery of policy and procedures. They will build effective working relationships with other health care professionals and work within the GP, nurse and health care assistant team to deliver primary health care services within the Practice.
Main duties of the job
* Venepuncture, handling of specimens
* Undertaking diagnostic tests and screening procedures within agreed practice protocols (e.g., ECG and cervical cytology)
* Immunisation and vaccination of children, adults and travellers
* Ear examination and syringing
* Wound care, dressings and removal of sutures
* First aid and management of emergencies (e.g., burns, haemorrhage, shock, cardiac arrest); triage of injuries
* Care and maintenance of equipment
* Maintain clear, concise and accurate documentation within the systems provided
* Ensure that all necessary arrangements are completed promptly and in a safe manner
* Ensure care is supported by best available evidence and local policies and procedures
* Deliver opportunistic health promotion where appropriate
* Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
* Uphold the confidentiality of all records/information held by Pembroke Medical Group; confidentiality lasts indefinitely and continues after employment; manage information in accordance with the Data Protection Act 2018
* Maintain professional competency and be in date for NMC revalidation

Job Description
Primary responsibilities include chronic disease management with diagnosis, monitoring and development of individual management plans; reviewing patients with chronic diseases (e.g., diabetes, hypertension, heart disease) to implement and evaluate treatment plans; assessing lifestyle and medication and suggesting changes; implementing evidence-based processes for long-term condition management; identifying and supporting patients at risk of developing long‑term conditions; signing and performing venepuncture, diagnostic tests and screening procedures (ECG and cervical cytology); immunisations and vaccinations; ear examination and syringing; wound care and sutures removal; first aid and emergency management; care and maintenance of equipment; new patient checks; safe signposting and comprehensive holistic assessments; maintaining documentation; effective communication with multidisciplinary teams; maintaining patient confidentiality; communicating complex information to patients/carers; promoting health and self-management; triage and assessment of patient needs; ensuring care is evidence-based and policy-aligned; safeguarding involvement; and keeping up-to-date with professional standards and Read coding.

* Qualifications (Essential): Registered nurse; membership of a professional body; evidence of CPD and training; evidence of continuous academic development
* Experience (Essential): Experience with clinical pathways; ability to work independently; phlebotomy; experience as a practice nurse in primary care; knowledge/experience of long-term condition monitoring; wound care management; immunisations; cervical screening; knowledge of QOF
