Job Description:
We are looking for a Qualified and Experienced Social Worker to join the CHAD team.
Working collaboratively with colleagues across agencies and council directorates from Health, Education, the voluntary sector, and Adult Services. You will have the opportunity to support children and young people with physical, sensory, and learning needs who may require Care and Support plans, Transition planning, Child Protection plans, Looked After Children procedures, and Court proceedings.
You will be committed to working with children with disabilities in a supportive, team-focused learning environment. As a growing team, we seek a confident individual who can think creatively and advocate for the needs of children and families.
It is essential that you are a qualified and experienced Social Worker registered or eligible for registration with Social Care Wales.

Responsibilities:
* Ensure the best outcomes for children and young people.
* Work within current legislation, council policies, procedures, and practices to assess, commission, and review services, including planned interventions for specific client groups.
* Undertake and promote effective assessment, care planning, intervention, and review for children in need of care and support, including those in care or protection, and when legal proceedings are necessary.
* Work with individuals, families, carers, and communities to help them make informed decisions, clarify needs, and contribute to service planning.
* Support children and young people through robust assessment, outcome-focused planning, and effective intervention to achieve positive outcomes within their families and communities.
Requirements:
* Social Care Wales registration
* A recognized Social Work qualification (Degree/DipSW/CQSW)
* A full UK driving license
* DBS clearance
* Authority to work in the UK
Salary scale: £38,000–£42,000 dependent on experience.
