This role will lead the delivery and development of highquality, personcentred pharmaceutical care, with a strong focus on polypharmacy reviews, care home medication reviews, and medicines management assessments across primary and community care settings. The Role As Lead Pharmacist, you will: Provide clinical and professional leadership to a pharmacy team delivering Realistic Medicineled polypharmacy reviews Lead and oversee medication reviews for patients with complex needs, frailty, or multimorbidity Drive the delivery of care home medication reviews, working collaboratively with GP practices, care home staff, and wider health and social care professionals Lead medicines management assessments, ensuring safe, effective, and personcentred use of medicines Embed Realistic Medicine principles, supporting shared decisionmaking and personalised care planning Contribute to service transformation, quality improvement, and implementation of national polypharmacy guidance Ensure clinical governance, prescribing quality, and risk management across services Support education, supervision, and professional development of pharmacists, pharmacy technicians and pharmacy support staff Use data and audit to demonstrate impact, outcomes, and continuous improvement You Will Bring Registration with the General Pharmaceutical Council (GPhC) Experience in polypharmacy reviews and working as an independent prescriber Experience of working across organisational boundaries in multidisciplinary teams Excellent communication skills and commitment to personcentred care Knowledge of NHS Scotland polypharmacy guidance and Realistic Medicine principles Experience in service development, quality improvement, or strategic planning would be desirable Why Join Us? Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
