Overview
The Discharge Lounge is an integral part of the hospital, supporting patient flow and facilitating safe and timely discharges to patients’ own homes or the next stage in their care journey. The role will include supporting the Registered Nurse (RN) with safe discharges and liaising with members of the multidisciplinary team (MDT) to ensure coordinated, patient‑centred care.
Responsibilities
* Promote dignity, respect and privacy in the delivery of care duties.
* Undertake patients’ vital signs, blood pressure, pulse, pulse oximetry, height, weight, neurological assessment, ECG, respirations, urinalysis, blood glucose and escalation of concerns in line with policy.
* Recognise deteriorating patient and follow escalation process.
* Provide delegated clinical procedures such as removal of cannula, wound assessment and simple dressings.
You must be able to work as part of a team, use your own initiative with effective interpersonal and communication skills, and contribute fully as a member of the multi‑disciplinary team. We will provide a supportive atmosphere, mentorship and clinical supervision on the unit and are committed to the professional development of all staff.
