Are you a motivated and experienced Registered Nurse with a passion for supporting older Australians? We are seeking a dedicated Office Based Registered Nurse to join our Home Care Package (HCP) Aged Care Team. In this pivotal role, you will provide clinical oversight, manage a team of Enrolled Nurses, and ensure effective delivery of high‑quality care across our HCP Program.
About the Role
As a Registered Nurse in our office based Aged Care team, you will:
* Provide leadership, supervision, and professional support to a team of Enrolled Nurses.
* Oversee the day‑to‑day management of the HCP program, ensuring compliance with the Aged Care Quality Standards.
* Conduct clinical assessments, review care plans, and monitor outcomes to ensure person‑centred care.
* Collaborate with clients, families, GP’s, and allied health professionals to support best practice.
* Drive quality improvement initiatives and maintain accurate clinical documentation.
* Assist the business and Aged Care team to transition to Support at Home.
About You
To be successful in this role, you will bring:
* Current APHRA registration as a Registered Nurse.
* Demonstrated experience in aged care, community care or HCP management.
* Strong leadership skills with the ability to mentor and manage a team of nurses.
* Excellent communication skills, problem solving, and organisational skills.
* Sound knowledge of Aged Care legislation, compliance and reporting requirements.
* Proficiency in care management systems and Microsoft Office Suite.
