Salary:£48,000 £64,000 per annum, dependent on experience
Contract:Full-time, Permanent
We are recruiting for a well known provider within the west midlands.
As an Advanced Nurse Practitioner (ANP), you will play a pivotal role in delivering high-quality, patient-centred care within our multidisciplinary team. Operating autonomously and collaboratively, you will assess, diagnose, treat, and manage patients presenting with a wide range of acute and chronic conditions. Your expertise will contribute to the development and implementation of evidence-based clinical guidelines, ensuring optimal patient outcomes and service delivery.
Key Responsibilities:
* Clinical Assessment & Diagnosis:Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.
* Patient Management:Prescribe medications (as an independent prescriber), initiate and manage treatment regimens, and provide follow-up care.
* Chronic Disease Management:Lead clinics for long-term conditions such as diabetes, hypertension, and asthma, implementing personalized care plans.
* Clinical Leadership:Provide mentorship and support to junior nursing staff, fostering a culture of continuous professional development.
* Quality Improvement:Participate in clinical audits, research initiatives, and service evaluations to enhance care quality and patient safety.
* Collaboration:Work closely with GPs, consultants, and other healthcare professionals to deliver integrated care pathways.
* Health Promotion:Develop and deliver health education and promotion programs to patients and the wider community.
* Record Keeping:Maintain accurate and up-to-date patient records in accordance with legal and professional standards.
Essential Qualifications & Experience:
* Registered Nurse with a valid NMC PIN.
* Masters degree in Advanced Clinical Practice (or working towards).
* Independent Prescriber status (V300).
* Minimum of 2 years' post-registration experience in a relevant clinical setting.
* Demonstrated competence in advanced clinical assessment and decision-making.
* Evidence of continuing professional development and reflective practice
