Responsibilities
* To care for patients and their families within their own homes.
* Collaborate with the district nursing team to deliver holistic care and support patient discharge planning.
* Provide care as part of a 24/7 service and participate in a robust induction programme.
* Develop clinical and interpersonal skills through continuing professional development opportunities.
Qualifications
* Registered nurse.
* Desirable ability to speak Welsh; Welsh and English speakers are equally welcome.
