Clinical Package Manager
to lead and oversee the delivery of high-quality, person-centred care for an individual with complex needs based in North Yorkshire (Harrogate, Whitby, Selby and Thirsk and surrounding areas.)
About the Role
As a Clinical Package Manager, you will provide clinical leadership and hands-on support, working closely with Team Leaders and frontline staff to ensure safe, effective, and compassionate care. This role involves delivering care within the individual's own home, the local community, and during hospital stays, while also collaborating with families and healthcare professionals to achieve the best outcomes.
Key Responsibilities
* Deliver and coordinate person-centred care, ensuring dignity, choice, and independence.
* Provide direct clinical care when required, including occasional weekend work.
* Develop, review, and implement clinical care plans.
* Oversee medication management in line with legislation.
* Lead on infection prevention and hygiene practices within the care package.
* Ensure compliance with Care Quality Commission (CQC) standards and internal policies.
* Work collaboratively with healthcare professionals to address clinical needs.
* Support and guide Personal Assistants and Nurses, promoting consistent high-quality care.
About You
* Registered Nurse (RN/RGN) or other qualified healthcare professional.
* Strong leadership and clinical assessment skills.
* Experience in community or home-based care (desirable).
* A person-centred approach, with a focus on dignity and respect.
* Excellent communication and organisational skills.
* Proactive, caring, and values-driven.
