Overview

Registered Nurse opportunity within Hughenden Valley Surgeries. Join a stable, experienced team across two sites serving a multicultural patient population. The practice is seeking a motivated, team-oriented nurse, ideally with chronic disease management experience (e.g., asthma and/or diabetes).

Main duties include treatment room responsibilities (including baby immunisations and cervical screening) and chronic disease management. The practice is a training environment focused on developing staff with clinical supervision, training, and support to grow professionally.


About Us

Main site: Hughenden Valley; sister site: Prestwood. Free staff car parking at both sites. The team comprises 4 GP Partners, 3 salaried GPs, a prescribing Paramedic, an Advanced Nurse Practitioner, and a nursing team with administration support. Hughenden Valley Pharmacy is located within the Hughenden site.

Job Details

Job title: Practice Nurse

Band: 6 (Agenda for Change). Salary: Depending on experience (competitive).

Contract: Permanent. Working pattern: Part-time, flexible.

Job Responsibilities

Accountable to: Lead Nurse (clinical issues) and Practice Manager (other issues).

Daily supervision: Self-managed Lead Nurse, Drs or Practice Manager for exception management.

Overall objective: Be an effective member of the team providing comprehensive nursing services to the Practice patients, while supporting GPs and other Healthcare Professionals.

* Provide patient care in a caring and efficient manner in line with Practice aims and values.
* Support GPs and Healthcare Professionals to enable them to fulfil their duties of care.
* Maintain patient and practice confidentiality at all times.

Communication and liaison: Effectively communicate with team members, patients and carers across both sites, internal staff, district nurses, health visitors, midwives, and others as needed.

Key duties and responsibilities

* Dressings, wound management, and related care (with GP/nurse referral when appropriate).
* Childhood immunisations in accordance with NHS guidelines and policy.
* Vaccinations, vaccines education, health and hygiene management; vaccines for foreign travel as appropriate.
* General health advice and health promotion; ECGs; blood pressure checks; venepuncture.
* Chronic illness management clinics according to practice and national protocols.
* Smears; removal of sutures/clips; IUCD/IUS fitting assistance; new patient health checks.
* Urinalysis and other routine nursing procedures; ensure NHS compliance for new patient checks.
* Ear examination with onward referral as needed; administration and professional responsibilities including accurate notes in the electronic patient record (EPR).

Training and development: Annual appraisal-based training requirements; staff development supported by the Practice; maintain up-to-date knowledge and revalidation requirements.

* Participate in education and training of students and induct new staff where appropriate.
* Continue education via courses and study days for professional development.

Equality and professional conduct: Support equality, diversity, rights of patients and carers; maintain privacy and dignity; be welcoming, non-judgemental and respectful of circumstances and rights.

* Actively contribute to quality and risk management; monitor own performance and accountability.
* Work with other agencies to meet patient needs and manage time, workload and resources efficiently.


Qualifications

Essential: Registered General Nurse (RGN) qualification and practice nurse experience.

Desirable: Diploma in Asthma care and/or Diabetes care.


Other Requirements

Disclosure and Barring Service (DBS) check is required. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; a disclosure submission will be made to the DBS.
