We are looking for a highly motivated, friendly, and organized Experienced Practice Nurse with a keen eye for detail who wants to make a difference to patients. The ideal candidate must be qualified in managing Long Term conditions such as Asthma, COPD, and Diabetes. We are seeking someone for 30-37.5 hours per week, who will deliver high-level, evidence-based, individual care to the Practice population. The post holder will be responsible for providing nursing services related to General Practice, including participating in patient care assessments, managing long-term conditions, treatment, preventative care, screening, and patient education. The role involves working at times without supervision.
Main duties of the job
The post holder will practice a range of essential clinical skills developed through experience and training, following Practice protocols. They will ensure clinical practice is safe, effective, and within their competence, acknowledging limitations such as:
* Assisting in/performing routine patient care tasks as directed by GPs
* Qualified in Chronic Disease Management - Diabetes, Hypertension, Asthma, and COPD
* Venepuncture
* Contraception
* Cytology
* ECG
* Blood Pressure Checks

* Assisting with IUCD fittings

Our team at Haslingden includes 2 GP partners, 1 salaried GP, 3 locum GPs, an ANP, Practice Nurses, and additional staff such as a First Contact Physio, Trainee Psychological Practitioner, Physician Associate, Pharmacist, Pharmacy Technician, and Health and Wellbeing Coach. Our admin team comprises a Practice Manager, deputy, and supportive staff including reception. We support staff learning and development.
Job responsibilities
This role requires consolidating specialist knowledge and skills in general practice nursing, demonstrating competence that supports evidence-informed, complex, autonomous decision-making. It involves clinical leadership, supervision, mentorship, and developing new models of service delivery, often with inter-professional and inter-agency approaches. The postholder will deliver care to the practice population, understanding public health profiles and needs, and proactively ensuring services match these needs. Responsibilities include:
* Assessment, screening, treatment, and health education
* Routine patient care tasks
* Pathological specimen collection and investigative procedures
* Administrative duties, maintaining confidentiality, accurate record keeping, and participation in practice meetings
* Chronic disease management and evaluation
* Liaising with team members and external agencies
* Participation in research, audits, and quality improvement initiatives
Training and personal development
Training needs will be monitored through annual appraisals. The postholder will complete mandatory training, develop an in-house training program, and keep up with recent developments. Personal development plans are encouraged. Collaboration with reception and admin staff is essential for smooth practice operations. Good liaison with other surgeries and agencies, including secondary care, is vital.
