Overview

We are looking to recruit an enthusiastic, caring and compassionate nurse to join the ILD service based at Worcestershire Acute Hospitals NHS Trust. The service covers the whole county with clinics at Worcester Royal, Alexandra Hospital, Redditch, Kidderminster Treatment Centre, Malvern community Hospital and BHI, Bromsgrove. The aim of the service is to provide high quality care and develop expert county-wide respiratory nurse services for adults with Interstitial Lung Disease (ILD).

Prevent any unnecessary hospital admissions/readmissions. Minimise length of stay and reduce barriers to safe discharge by facilitating safe installation of oxygen as appropriate. Ensure safe provision of oxygen in accordance with BTS (2015) guidelines. Ensure safe provision of ventilation at time of initiation and review. Ensure safe guideline-led DMARD monitoring cross county. Take responsibility for development, implementation and evaluation of this specialist service. Provide a link to tertiary centre antifibrotic therapy and lung transplantation services.


Responsibilities

* Promote the role of the specialist respiratory nurse in order to deliver an effective service embedded in inpatient and outpatient service delivery models.
* Provide cross-county specialist respiratory nurse-led ILD clinics including: oxygen clinic, DMARD initiation and monitoring protocol.
* Participate in the prospective cover (1 in 8) general weekend rota to provide support to the wards for oxygen, asthma and NIV discharges.
* Deliver joint respiratory consultant and nursing clinics for ILD.
* Support/cover the Band 7 in a coordinating and administrative role in the ILD MDT meeting and act as the patient’s advocate when required.
* Educate and support patients at the time of diagnosis.
* Provide patient-initiated phone support and remote management.
* Provide a link by referral to pulmonary rehabilitation and palliative care services.
* Support the Band 7 Clinical Specialist to develop nurse-led follow up for stable IPF and shared care appointments for patients receiving antifibrotics from tertiary centres.
* Provide cross-county specialist respiratory nurse-led clinics including: oxygen clinic, ventilation clinics, nebulised antibiotics, urgent ad-hoc clinics.

Job responsibilities

General Duties:

Educational:

Develop and deliver specialist training and educational programs for nurses, doctors, physiotherapists, midwives and students, related to respiratory care, to develop the skills required to deliver safe respiratory care and promote a wider understanding of oxygen and its treatment. Proactively assess the learning needs of health professionals and identify, develop and deliver training packages to meet these needs. Act as an expert resource for health care professionals working with respiratory patients within the primary and secondary care settings, to advise about appropriate delivery of care. Identify the educational needs of the specialist group of patients and their carers and ensure these are met through the service provided. Forge links with education providers and contribute to respiratory-related educational programmes within the Trust. Identify and promote respiratory care-related learning opportunities for other nursing staff.

Patient / carer Support / Involvement:

Provide education and advice for people with respiratory disease to help them understand the disease process, its management and coping strategies. Provide emotional support for patients across the disease course and during the final stages. Support patients, families and carers to participate in decisions concerning their care by providing information on available services. Act as an expert resource for carers of asthma patients and develop written information leaflets for patients, liaising with clinical governance and the patient information group.

Leadership and Management:

Develop the specialist respiratory nursing service and act as a role model and support for staff on respiratory and medical wards. Actively participate in multidisciplinary ward meetings and provide clinical leadership. Promote best practice using benchmarking groups to develop standards and clinical guidelines. Liaise with hospital departments on respiratory issues and participate in relevant groups representing the Respiratory department.

Personal / Professional Responsibilities:

Maintain ongoing professional development in line with NMC requirements, keep patient management records per NMC guidelines and Trust standards, and promote good working relationships to advance the specialist service and patient-centred care.

Research and Audit:

Stay up to date with research and audits, critically appraise outcomes, and participate in local and national respiratory research in line with Good Practice Guidelines and local policy.

Clinical Governance:

Act in accordance with the NMC code of conduct and local policies, contribute to guideline development, and disseminate national and local guidelines to staff to ensure evidence-based care.

Strategic / Service Development:

Contribute to the operational and strategic development of the respiratory nursing service, participate in strategic health initiatives, and apply Quality Improvement techniques to service development.


Qualifications

Essential: Registered Nurse; currently registered with the relevant UK professional body (NMC/HCPC); meets CPD standards. Desirable: evidence of coaching and supporting learners in practice (practice-based assessor/supervisor); strong commitment to ILD specialty through CPD and clinical experience.


Experience

Essential: minimum 2 years post-registration experience including respiratory medicine; evidence of CPD; knowledge of current issues in acute health service; awareness of national/local targets; post-qualification training in respiratory care; evidence of participation in research/audit; experience of staff supervision and development. Desirable: experience in care of patients requiring non-invasive ventilation; oxygen assessment; care for patients with ILD.


Personal Qualities

Essential: excellent interpersonal skills; ability to manage change; handle stressful situations; leadership, motivation, delegation, and teamwork; problem-solving; patient-centred service mindset. Other essential: able to travel between sites; flexible to service needs; ability to perform moderate to intense physical effort and concurrent activities.
