An exciting new nursing opportunity has been developed in Evelina community for an experienced Health Visitor for children with Complex needs.

The nurse must hold a SCPHN qualification, having experience in the assessment and management of pre-school children with complex needs and their families as part of a multidisciplinary team

An exciting new nursing opportunity is available in the Evelina community for an experienced Health Visitor specialising in children with complex needs.

The ideal candidate must hold an SCPHN qualification and have experience in assessing and managing preschool children with complex needs and their families, working as part of a multidisciplinary team within the Community Services Directorate.

This is a fixed-term/Secondment contract for 12 months.

Main duties of the job

You will need to be highly motivated, self-directed and work autonomously, managing a caseload of families and children within the speciality whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care.

You will be working within the Community Paediatricians team in both Lambeth and Southwark. You will be expected to work flexibly across both boroughs and see patients at Sunshine House and the Mary Sheridan Health Centre.

Detailed job description and main responsibilities

To work autonomously, managing a caseload of families and children within the speciality whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care.

Act as an advocate for children with special needs within the department, to ensure a client orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.

Provide skilled continence assessment and advise families and other professionals on the promotion and management of continence in children.

Ensure that CYP receive high-quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.

Recognise and avoid situations that may be detrimental to individuals' health and well-being.

Actively engage service users through the facilitation of user groups and open days.

Coordinate the provision of specialist services to children and families both within hospital and/or in the community setting, effectively liaising with multidisciplinary colleagues.

Participate in the promotion of patient centred care and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and aftercare.

Support families and carers to make informed decisions relating to their treatment and management of their children's care.

Promote high-quality clinical care and assist in the development of the specialist service through the use of evidence-based practice and clinical standards.

Provide professional and clinical leadership and expertise in the speciality, and act as a resource for education and clinical expertise in the hospital and community settings.

Demonstrate appropriate expert practice and highly specialist advice in the assessment, planning, implementation, and evaluation of patient care.

Promote and deliver high standards of care and provide clinical leadership in the specialist areas.

Professional/Statutory Registration

Essential criteria

1. RN1 or RN Child Registration Nurse, HV SCPHN on the NMC register. To hold the appropriate level for the area of speciality.

Qualifications/Education

Essential criteria

2. Equivalent qualification/training and experience
3. Your professional knowledge acquired at degree level will be supplemented by specialist training, experience and short courses to Masters level equivalent.
4. Post registration qualification in appropriate area of speciality or equivalent post qualification experience.
5. Leadership course
6. Mentorship course

Desirable criteria

7. Working towards a Masters degree

Previous experience

8. Extensive experience of working autonomously at an advanced level within the specialist area.
9. Delivering effective patient-focused care.
10. Proven evidence of management / leadership skills and autonomous practice.
11. Previous experience of student supervision and clinical education at Undergraduate level.
12. Experience of delivering patient advocacy.
13. Experience of delivering change management both personally and as a facilitator.

Skills/ Knowledge/ Ability

Essential criteria

14. Advanced clinical, managerial and leadership skills in a speciality.
15. Proven listening / counselling skills with the ability to manage complex situations and appropriately deliver sensitive and difficult outcomes.
16. Evidence of teaching skills, in particular, teaching patients how to manage their conditions and raise their awareness of their condition.
17. Ability to clinically lead & influence staff.
18. Up-to-date knowledge of current clinical and professional issues
19. Sound knowledge of current issues in the profession and practice of nursing.
20. Excellent verbal, written, interpersonal & communication skills.
21. Able to use skills of history taking and physical assessment to direct patient care.
22. Time management and organisation skills
23. Ability to conduct & evaluate own projects successfully.
24. Demonstrate an understanding of: - the National agenda for speciality.
25. Knowledge of quality standards & NICE recommendations/NSF
26. Ability to carry out audit and research.
