Job Summary
We are seeking a B6 Frailty Nurse to join our Assessment and Care Team. Working at a PCN level, the AC Team supports frail, housebound patients and care home residents registered at our four GP surgeries. This post is for 22.5 hours a week.
Responsibilities
Key duties include:
* Supporting patients to live safely and independently at home.
* Reducing admissions to hospitals.
* Conducting weekly calls to care homes for resident reviews.
* Making referrals to external agencies such as Adult Social Care and voluntary sector services.
* Annual reviews and long‑term condition monitoring.
* Administering vaccinations (flu, COVID‑19, shingles, pneumonia and RSV).
* Developing care planning and treatment plans.
* Performing health assessments in a community setting, usually in the patient’s home.
Qualifications
Essential
* Adult Nursing Registration
* Phlebotomy competence
* Immunisation training
* Knowledge of DNAR forms
* Own vehicle for visiting patients
Desirable
* Experience with SystmOne
Contract & Compensation
Salary: £20.44 – £24.61 per hour (pro‑rated for part‑time). Permanent contract. Working pattern: part‑time, flexible – 22.5 hours per week, which can be arranged as school‑friendly hours across five days or as three standard days.
