This role will be working within the Older people's Community Mental Health Team within Bradford. The post will require the suitable candidate to be able to access a wide Bradford area due to a mixture of seeing patients at clinics and community settings.
Access to transport will be an asset due to the carrying of the physical health monitoring equipment.

* To support the Community Mental Health Team (CMHT) in undertaking detailed assessments of ability for service users accessing the team.
* To undertake physical health assessments and reviews of services users and promote healthy living and lifestyle.
* Support the delivery of therapeutic Interventions.
* Working alongside service users, family members and carers to provide signposting, support and advice.
* Working in an interdisciplinary way to provide person-centred holistic care and promote independence for individuals.
* To contribute to the Memory Assessment and Treatment Service (MATS) with on-going evaluation, data collection and service development.
