What the Job Involves You will deliver a high-quality social work service to vulnerable older people and their families, promoting independence while providing necessary support. This involves partnering with citizens, carers, and various agencies to identify needs, ensure rights are safeguarded, and meet statutory timescales for assessments and reviews. You will use person-centered planning approaches, respond to safeguarding concerns, and contribute to multi-disciplinary meetings. The role includes maintaining a caseload, using evidence-based practices, and promoting anti-discriminatory conduct.
