Care Co-ordinator - Community Perinatal Mental Health Team

Specialist Mother and Baby Mental Health Service (SMABS) is a community perinatal mental health service commissioned with the Bradford District Care NHS Foundation Trust (BDCFT). We cover the catchment area of Bradford, Airedale and Craven, with approximately 8,000 live births per year – the main maternity hospital sites being Bradford (6,000 births) and Airedale (2,000 births). We are looking to recruit an experienced, highly motivated and enthusiastic mental health professional/practitioner with a specialist interest in improving care for women and their families in the area. Experience in Perinatal Mental Health is desirable; a keen interest in this specialist area is essential.
SMABS provides mental health care and treatment for women and their families from pregnancy through the first year post‑delivery. The service is multi‑disciplinary and supports women experiencing moderate to severe mental ill health or a high risk of relapse during pregnancy and the perinatal period. Practitioners provide care coordination, assessments and maintain a duty service that offers advice, information and signposting regarding perinatal mental health.
Main duties of the job
* Complete a holistic triage for referrals coming into the service.
* Work closely with maternity services (midwives, health visitors) and collaborate with other providers to deliver personalised care plans for people during pregnancy, birth and postnatally.
* Provide assessment and treatment aimed at enhancing the parent‑infant relationship, ensuring the emotional and physical well‑being and development of the infant.
* Serve on the duty service providing advice, information and signposting on perinatal mental health.
Perinatal Mental Health is a rapidly expanding area with significant investment, offering high‑quality training and career development opportunities. Practitioners have access to a comprehensive induction package and specialist training both internally and externally to develop high‑level expertise in perinatal mental health conditions, risks, relapse factors and evidence‑based interventions. Additional learning in infant mental health, development, attachment and bonding is also available.

Job responsibilities
* Carry out holistic triage for all referrals.

Person Specification
Qualifications
* Professional qualification (RMN, Social Work, Occupational Therapy) to degree level or equivalent.
* Current registration with the NMC or HCPC.
* Minimum two years post‑basic training.
* Experience in a relevant community setting of adult mental health service.
* Psychological therapy qualification in a relevant modality.
Experience
* Significant experience in a mental health setting.
* Experience of working with Perinatal Mental Health problems, substance misuse and/or emotional dysregulation.
Knowledge and Skills
* Assessment and care planning skills.
* Key worker case management/caseload management.
* Understanding and application of the Care Programme Approach (CPA), the Mental Health Act and safeguarding policies and procedures.
