Are you an experienced Nurse or Matron who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 7 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing and leading efficient, integrated, urgent care to our patients with care and respect.

Main duties of the job

You will be working as part of the Primary Care at Home team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage, prescribing (NMP qualification is essential for this role), and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community. You will deputise for the Primary Care at Home or Service Manager as required to ensure Unplanned Care pathways are delivered to a high standard.

Part of this role will include covering our EEAST Ambulance Stack and Falls Collaboration Car with Bedfordshire Fire Service as required.

Detailed job description and main responsibilities

We are looking for an enthusiastic and motivated nurse with community experience, who is passionate about delivering high quality care. Excellent communication, time management skills and sound clinical knowledge, is essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

The Trust aims to recruit and retain the best possible staff, who will make a positive contribution to the service, in return we offer excellent development opportunities and training that can be directed towards your needs/goals. You will have regular supervision, mentorship and support from within the Primary Care at Home Team and the Trust.

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

Education/Qualifications

Essential criteria

1. Registered General Nurse
2. Educated to degree level or equivalent
3. Post-registration qualification in a relevant clinical field
4. Nurse Prescriber
5. Relevant teaching/mentoring qualification and evidence of teaching skills
6. Demonstrable evidence of significant post-registration nursing experience, including in a community setting

Experience

Essential criteria

7. Evidence of and commitment to multi-professional working
8. Demonstrate working within a multi-agency environment.
9. Have a comprehensive understanding of partnership working
10. Demonstrate application of clinical governance
11. Excellent understanding of community health and social care principles and practice
12. Understanding of National and Local issues that impact on the population of Bedfordshire.
13. Comprehensive understanding of the roles of others in health and social care
14. Experience managing multi-disciplinary staff
15. Experience in managing issues related to conflict resolution avoidance
16. Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
17. Knowledge of the principles of health promotion and prevention of illness
18. Experience of using electronic systems to manage activity and the delivery of KPI information
19. Evidence of specialist holistic assessment and evaluation skills
20. Ability to prioritize and manage own time to meet the needs of the service
21. Ability to collect and interpret data in relation to patient activity and nursing care
22. Demonstrate experience in monitoring of staff and quality of service provided

Knowledge and skills

23. Demonstrate evidence-based practice and up-to-date clinical skills
24. Experience of clinical leadership
25. Extensive experience of complex assessment of health and social needs
26. Ability to oversee multiple caseloads and monitor quality.
27. Demonstrate excellent communication skills using a variety techniques and strategies
28. Ability to disseminate complex sensitive information to others
