What you will do
* Assessment and Discharge Planning: Complete social work assessments for adults in hospital and coordinate safe, timely discharges in line with legislation and policy.
* Safeguarding and Risk Management: Identify and manage adult protection concerns, complete risk assessments, and contribute to protection planning while promoting independence.
* Multi-Disciplinary Working: Work closely with NHS professionals including doctors, nurses, and therapists, attending ward rounds and discharge planning meetings.
* Legal and Policy Frameworks: Apply relevant Scottish legislation, including the Social Work (Scotland) Act 1968 and Adults with Incapacity (Scotland) Act 2000, and contribute to Mental Health and guardianship processes where required.
* Recording and Professional Practice: Maintain accurate and timely case records and engage fully in supervision, training, and continuous professional development.
Requirements
* Qualification: Degree in Social Work or equivalent recognised qualification.
* Registration: Registered with the Scottish Social Services Council (SSSC).
* Experience: Proven experience in adult social work or health-related settings; hospital discharge or integrated team experience is highly desirable.
* Skills: Ability to manage complex cases, work under pressure, and collaborate effectively within multidisciplinary teams.
