 
        
        An opportunity has arisen within the ICB for colleagues who are qualified as RMN/RNLD clinical role to consider a change in direction and join the admission avoidance team.
We are looking to recruit a dynamic and innovative Band 7 admission avoidance manager to work as part of the team, chairing care education and treatment reviews (CETR), ensuring that the person is at the centre of their care and treatment.
The post holder will work with individuals with learning disabilities and or autistic people and their families, both within hospital settings and or within the community to seek solutions that support people to remain living safe and well within their communities and avoid admission to hospital.
The post holder will ensure that the CETR is carried out in a manner that is based on the person-centred values laid out in the national CETR policy., The post holder will be employed by Humber and North Yorkshire ICB, as part of the admission avoidance team, supporting the Learning Disability and Autism agenda across all Places within the geography (e.g. North Yorkshire and York and Humber).
They will work closely with members of the ICB Case Managers and Local Authority Social care managers. The post holder will lead on the continued implementation of the revised national Care Education & Treatment Review (CETR) Policy and Guidance (March 2017), undertaking community and inpatient CETRs and providing specialised advice for people with learning disabilities and/or autism who display behaviours which challenge under the Transforming Care Partnership programme.
The postholder will provide a chair function for both community and inpatient C(E)TRs providing highly specialised advice for people with learning disabilities and/or autism who display behaviours which challenge under the Transforming Care Partnership (learning disabilities and austism) programme.
Working as part of a Admission Avoidance Team, the postholder will work with other CTR and CETR Lead colleagues to develop processes and working practices which will form a cohesive approach to C(E)TRs, LAEPs and Dynamic Support Registers across The ICB.
